Infant Gut Microbiome at Birth: Why Early Colonization Matters

Jarvinen-Seppo opens by describing how rapidly the infant gut microbiome develops in the first year of life, continuing to mature until roughly age 3. She emphasizes that the earliest months are especially consequential because they set the trajectory for how the microbiome will develop over time, and because gut microbial development is closely intertwined with the maturation of the immune system. She describes a significant interplay between the infant gut microbiome and the types of immune responses that develop in the gut mucosa during this window.

Asked which nutritional and environmental factors shape the microbiome earliest, Jarvinen-Seppo points first to feeding type. Whether an infant is breastfed, formula-fed, or mixed-fed has a substantial impact on microbiome development, largely because human milk oligosaccharides (HMOs) in breast milk support heavy colonization of bifidobacteria and other taxa well-equipped to use HMOs for growth. She identifies antibiotic use as another major factor, noting that certain bifidobacteria are highly susceptible to antibiotics, so antibiotic exposure can wipe out these beneficial commensals. Delivery mode and place of delivery — home versus hospital, cesarean versus vaginal birth — also influence colonization, though Jarvinen-Seppo notes that recent data suggest this effect is smaller than once thought relative to feeding type and antibiotic use. She adds that complementary feeding, once introduced, has a significant effect, as does the broader household environment — for example, infants with more older siblings tend to show different microbiome development.
